What is Cardiovascular disease?

Cardiovascular diseases (CVDs) are leading causes of death globally. Individuals at risk of CVD may have raised blood pressure, raised glucose levels, high cholesterol or be overweight or obese.

Four lifestyle factors increase the risk of fatal complications of CVDs. They include tobacco use, physical activity, an unhealthy diet and increased alcohol use.

The Cardio Care Programme

The Cardio Care Programme is designed to offer members optimal cover that ensures the best quality care and outcomes. The Cardio Care Programme enables Your Premier Plus GP to diagnose and initiate appropriate treatment while managing your risk factors with the support of a high functioning multidisciplinary care team.

How to access to the Cardio Care Programme

You must consult with a Premier Plus GP to access the Cardio Care Programme, as well as be registered for at least one of the following conditions as part of the Chronic Illness Benefit (CIB):

  • Hypertension
  • Ischemic heart disease
  • Hyperlipidemia.

Note to KeyCare and Smart plan members:

  • Members on KeyCare plans must choose a doctor who is on both the KeyCare and Premier Plus networks
  • Members on Smart plans must choose a doctor who is on both the Smart and Premier Plus networks

As part of the Cardio Care Programme, you and your Premier Plus GP can track your progress on a personalised dashboard that will help to identify the next steps you should take to manage your condition and stay healthy over time.
