Physical and virtual 24-hour care delivery facilitated by a dedicated care team that includes doctors, nurses and allied healthcare professionals.
Remote monitoring
24/7 real-time remote monitoring supported by cutting-edge digital healthcare technology
A patient's condition is monitored 24/7 through cutting-edge remote monitoring technologies. These send data to a secure dashboard that is monitored by the patient's Discovery Home Health nurse. Discovery care co-ordinator and team of clinicians in the ER Consulting command centre. You can also view your patient's vitals on-demand, on the secure dashboard.
Hospital-level care
Hospital-level diagnostics and interventions
Patients get access to a range of clinical diagnostic procedures and interventions to manage medical or postsurgical conditions with hospital-level care in their home. We support this with extra benefits for which we pay from risk (not the patient's day-to-day benefits) to improve the member's experience.
