Definitions

Discovery Connect Distribution Services (Pty) Ltd ("Discovery Connect"), registration number 2015/173165/07 an authorised financial services provider and offers the medical plan recommender tool.
Discovery Health Medical Scheme ("Scheme"), registration number 1125 is the medical scheme.
This is a non-profit organisation, registered with the Council for Medical Schemes.
Discovery Health (Pty) Ltd, registration number 1997/013480/07 ("Administrator") is a separate company and an authorised financial services provider and is the administrator for the Scheme.
Discovery Insure (Pty) Ltd, registration number 2009/011882/06 is a separate company and an authorised financial services provider which provides Gap Cover.
Discovery limited ("Group"), registration number 1999/007789/06 includes subsidiaries. Subsidiaries in the Group are authorised financial services providers.

The online quote recommender tool is provided by Discovery Connect for Scheme and Group products.

The information is about a number of Discovery Health Medical Schemes plan types that are most likely to fit the healthcare needs you highlighted. This is not a recommendation of the most appropriate plan type for you and it is not financial advice.

The information only gives you an idea of the contributions for the Scheme plans. It is also just a high-level benefit summary for a range of plan types that may fit your needs. The information does not include any possible underwriting, waiting periods or late-joiner penalties. These will only be assessed once you submit your application.

By submitting this request, you give your personal information to Discovery Connect. You also give Discovery Connect permission to let a registered financial adviser to contact you to do a needs analysis. The financial adviser will also discuss the different plan types available to you in detail and help you complete and submit your membership application to the Scheme.

Privacy statement ("Statement")

You give Discovery Connect permission to collect, store and disclose certain information relevant to your application with the Scheme, Administrator and Group..

This Statement explains how Discovery Connect, Scheme, Administrator and Group obtain, use, disclose and otherwise process personal information, which may include health and financial information. You give us permission to collect, collate, store and disclosure the information you give us. This is for any purpose that has to do with giving you sound and proper financial advice. Any additional information you give the financial adviser is for:

• Doing a full needs analysis to find out what your financial needs are
• Completing comparison quotations
• Getting information from the Financial Services Exchange (Astute) or any other financial institution
• Administration purposes and fraud prevention. It is also for offering you Group-wide services, benefits and infrastructure.

The financial adviser will treat all private information as confidential. Authorised users may not make your privation information public without your written permission.

Consent to share policy information with the Financial Adviser

By acknowledging the content of this notice, you authorise Discovery Connect to share your information (including your health information) with your chosen / appointed financial adviser during any underwriting process.

General

This Notice explains how a representative of Discovery Connect can obtain, use and disclose your personal information. This is in line with the Protection of Personal Information Act, No. 4 of 2013 ("POPI").

• You have the right to ask for copy of the personal information we hold about you. To do this, complete the Data Subject Request form at www.discovery.co.za and tell us what information you would like. We will take all reasonable steps to confirm your identity before providing details of your personal information.
• To get the information, you may pay have to pay a fee.
• You have the right to ask us to update, correct or delete your personal information. You can do this by contacting us.
• Please note that we may change this Notice from time to time.
• You have to accept these terms and conditions so that we can provide financial services to you.
• If you are acting on behalf of a minor, incapacitated person or a person unable to act on their own, you have to confirm that you have the authority to do so.

Discovery Gap Cover disclaimer

Discovery Gap Cover is an insurance product. This is not a medical scheme and the cover is not the same as that of a medical scheme. The policy is not a substitute for medical scheme membership. Discovery Gap Cover is a short-term insurance policy, underwritten by Discovery Insure Ltd, registration number 2009/011882/06, an authorised financial services provider. This is a separate short-term insurance product and is not conditional on the purchase of the long-term insurance Discovery Supplementary Gap Cover product. Product rules, terms and conditions apply.
