This introduction to Discovery Health Medical Scheme (the Scheme) is provided to you by Discovery Health (Pty) Ltd ("Discovery"), as administrator to the Scheme. Discovery is a registered financial services provider (FSP No 18564), licensed to provide health service benefits.

This information is provided to you by Discovery Connect Distribution Services (Pty) Ltd ("DCDS"), as a brokerage, on behalf of Discovery Health. DCDS is a registered financial services provider (FSP No 46336), licensed to provide advice and intermediary services.

The information we give you is factual information for a number of plan types - registered by the Scheme - that is most likely to align to the key considerations you have highlighted. This should not be considered as a recommendation of the most appropriate plan type for you. It should also not be deemed to represent the rendering of any financial advice to you.

The information only gives you an indication of the contributions payable and a high-level benefit summary for a range of plan types that may be appropriate to you. It does not take into account any possible waiting periods or late-joiner penalties that the Scheme may impose. These will only be assessed once you submit your application.

By submitting this request, you give your personal information to Discovery. And you give Discovery permission to instruct a DCDS registered representative to contact you to conduct an appropriate needs analysis. The representative will also discuss the different plan types available to you in detail and help you to complete and submit your membership application to the Scheme.

Whenever you use the Site, complete an application form, contact Discovery electronically, or use one of the products, services, facilities, tools or utilities offered by Discovery on the Site, Discovery will collect your personal information.
