What is the Population Health Booster?
The Population Health Booster is Discovery Health's supplementary reimbursement model and practice support tool that helps GPs measure, strengthen and sustain best-practice care. Built around actions and behaviours recognised by the profession, it supports better patient outcomes while creating new opportunities for practices to be rewarded for the value they deliver.
Designed with doctors, for doctors, it rewards healthcare professionals for supporting patients more by integrating HealthID actions, Personal Health Pathways interactions, Care Programme enrolment and chronic condition registration into one seamless system.
Population health Pathways is currently available to Discovery Health Medical Scheme members aged 18 years and older.
